You and your dependents are covered up to $5,000 per accident, after only a $100 deductible. Benefits
include expenses for medical, dental, chiropractic, ambulance, emergency room, and more. Worldwide
benefit covers accidents outside the United States. Benefit is available per person, per occurrence, with no usage limits. Also includes accidental death coverage of $10,000.

• Covers all family members (family is defined as immediate family members, spouse and children up to age18 or 23, if a full-time student).
• Benefit is available per person, per occurrence, per family member.
• The member can pay the bill at time of service and be reimbursed by the insurance company OR the member can pay the $100 deductible, and have the provider (hospital, physician, dentist or health care facility) bill the insurance company directly for the services provided.
• The included worldwide benefit can be used for accidents that occur out of the United States.
• Accepted at emergency rooms, clinics, urgent care centers, dental offices and physician's offices. Ambulance services are also covered.
• You will have coverage of $5,000 of Accidental Death. Your spouse is automatically insured for 50% of your coverage and each dependent child is automatically insured for 29% of your coverage.
• Members receive a Certificate of Insurance that will detail the specific level of coverage provided with enrollment.
